Exemestane Description1,2
Exemestane is a steroidal aromatase inhibitor that is structurally related to androgens such as androstenedione and testosterone. It irreversibly inhibits the aromatase enzyme, thereby reducing estrogen production. Exemestane is used in postmenopausal women for the treatment of advanced breast cancer as well as estrogen receptor–positive early-stage breast cancer.

Aromatase activity is irreversibility inhibited by exemestane. It reduces estrogen (estrone, estradiol and estrone sulphate) levels by approximately 90%. The enzyme aromatase is critical in converting androgen from the adrenal gland and ovaries to estrogens. The drug is structurally like androgens which provides the attraction for binding to aromatase. The process of binding leads to “suicide inhibition” of the drug when binding irreversibly to the enzyme. With less estrogen production hormone-dependent breast cancer cells regress.
