Dehydroepiandrosterone (DHEA) is an endogenous steroid hormone produced mainly by the adrenal cortex, with smaller amounts synthesized in the gonads and the brain. It acts as a precursor to both androgens and estrogens, including testosterone and estradiol. DHEA levels naturally decrease with age, and supplementation is used in contexts such as age-related hormonal decline, adrenal insufficiency, mood support, and sexual health.

Transdermal DHEA may be compounded into a cream or gel for application to the skin, enabling systemic absorption while bypassing first-pass hepatic metabolism. This route of administration may help minimize side effects and support more consistent hormone levels compared with oral DHEA.

Mechanism of Action1-3

DHEA is a prohormone, meaning its physiological effects are largely mediated through conversion to active androgens (like testosterone) and estrogens (like estradiol).

Endocrine effects:

In peripheral tissues, DHEA is converted to androstenedione, then to testosterone, estrone, and estradiol via tissue-specific enzymes such as 3β-HSD, 17β-HSD, aromatase.

Neurosteroid activity:

DHEA may interact with GABA-A and NMDA receptors, contributing to its neuromodulatory, antidepressant, and anxiolytic properties.

  • Hormone-sensitive cancers (e.g., breast, prostate, uterine)
  • Pregnancy and breastfeeding (safety not established)
  • Known hypersensitivity to DHEA or transdermal base formulation

Precautions1-3

  • May exacerbate androgen-sensitive conditions (e.g., acne, PCOS)
  • Monitor in individuals with bipolar disorder or psychiatric history due to possible mood effects
  • May interfere with hormone testing or HRT regimens
  • Caution in individuals with liver dysfunction, though transdermal avoids first-pass effect
