Tretinoin1,2

Tretinoin is a naturally derived form of vitamin A, also known as all-trans-retinoic acid. Retinoids may support the regulation of cell reproduction, proliferation, and differentiation. Tretinoin is believed to promote skin cell turnover, which may enhance skin texture and tone. It is used in the management of acne vulgaris as well as photoaging concerns, including facial wrinkles, skin roughness, and hyperpigmentation. In hyperpigmentation disorders, tretinoin may inhibit the transcription of the enzyme tyrosinase, thereby reducing melanogenesis.

 

Coenzyme Q10 (CoQ10)3-5

Coenzyme Q10 (CoQ10) is a lipophilic compound naturally present in the body and plays a key role in cellular energy metabolism via the mitochondrial electron transport chain. It also acts as an antioxidant that helps neutralize free radicals. This antioxidant activity and involvement in mitochondrial processes may help reduce visible signs of skin aging and support the maintenance of skin elasticity associated with aging. CoQ10 levels decline with age, which may result in decreased protection against reactive oxygen species.

The combination of tretinoin and CoQ10 supports skin cell turnover while enhancing antioxidant activity, contributing to improved skin integrity.

Tretinoin binds to retinoic acid receptors (RARs) alpha, beta, and gamma along with retinoid X receptors (RXRs) to block inflammatory mediators. In addition, during the binding to the retinoid acid receptor, the production of procollagen increases to augment collagen type I and III formations.

CoQ10 has a critical role as a cofactor for mitochondrial enzyme pathways within cells. Impairments in mitochondrial function disrupt cellular adenosine triphosphate (ATP) production and thereby inhibit the fuel for cellular repair mechanisms and function. Reactive oxygen species accumulate, which in turn can damage mitochondria, cellular membranes, and mitochondrial DNA, accelerating skin aging in a negative feedback loop. Coenzyme Q helps to facilitate mitochondrial activity and maintain energy. CoQ10 inhibits the ability of free radicals to activate mitogen activated protein kinase pathway and the ability of collagenases to form minimizing damage to collagen. It also decreases damage caused by lipid peroxidases to li[pids, proteins and DNA.

  • Burning sensation, irritation, erythema
  • Do not use in patients with open wounds
  • Photosensitivity with tretinoin
  • Initial “retinoid reaction” is common and usually subsides with continued use.

 

CoQ10 3-5

  • Generally well-tolerated in topical formulations.
  • No major adverse effects reported in clinical studies

Contraindications1,2

  • Hypersensitivity or intolerance to tretinoin or CoQ10
  • Presence of active dermatitis, eczema, or open wounds at the application site
  • Avoid use in pregnant patients

Precautions1,2

  • Use with other agents that can dry or irritate the skin (benzoyl peroxide)

Store at controlled room temperature (20–25°C / 68–77°F). Protect from excessive heat and direct sunlight. Keep the container tightly closed. Do not freeze. Keep out of reach of children.
