Micronized estriol is a bioidentical form of estriol (E3), one of the three naturally occurring estrogens in women, along with estradiol and estrone. Estriol is considered the weakest estrogen, exhibiting lower receptor binding affinity and a shorter duration of action compared to estradiol (E2). Despite its lower potency, estriol has demonstrated potential skin-related effects, including support of dermal thickness and elasticity. It is commonly used in combination with estradiol in postmenopausal women with estrogen deficiency. Estriol is most often compounded in a transdermal cream, which allows for gradual dose adjustments.

Aloe vera is derived from the gel found within the leaves of plants typically grown in hot, dry regions such as Africa, Asia, and the Canary Islands. It has been utilized for its pharmacological and cosmetic properties. Therapeutically, aloe vera exhibits systemic activity, including digestive support and properties associated with anticancer and antidiabetic use, and is applied topically for its antioxidant and anti-inflammatory effects. These properties contribute to skin protection and support wound healing. Aloe vera gel contains approximately 75 compounds, including vitamins (A, C, E, and B12), enzymes (amylase, catalase, and peroxidase), minerals (zinc, copper, selenium, and calcium), sugars (mannose-6-phosphate and glucomannans), anthraquinones, fatty acids, and hormones.

Estriol acts by binding to estrogen receptors alpha (ERα) and beta (ERβ) in estrogen-responsive tissues, particularly the vaginal epithelium, bladder, urethra, and pelvic floor musculature.

Aloe vera has several major compounds including aoesin, aloin, and emodin. These compounds are thought to have protective properties when applied topically through antioxidant and anti-inflammatory activities. These properties on the skin are facilitated by aloe vera’s ability to upregulate TFGβ1, bFGF, and Vegf-A expression in fibroblasts and increase keratinocyte proliferation and differentiation by lysosomal membrane stability. It also increases type IV collagen. Aloin is believed to protect the skin through its antioxidant properties by decreasing IL-8 production, DNA damage, and reactive oxygen species formation. It exerts anti-inflammatory activity by inflammatory cell infiltration, increasing CD4+/CD8+ ratio lymphocytes, and improving epidermal thickness and collagen deposition.
