BLT (Benzocaine / Lidocaine / Tetracaine) Description1-5
Benzocaine / Lidocaine / Tetracaine (BLT) Cream is a compounded topical formulation that combines three anesthetic agents and is used prior to dermatologic procedures such as laser treatments, tattooing, microneedling, and dermal filler injections. Local anesthetics applied to the skin are classified as either amino amides or amino esters. Lidocaine belongs to the amino amide class, while benzocaine and tetracaine are categorized as amino esters.
Benzocaine has an onset of action of approximately one minute, with effects lasting about 15 to 20 minutes. Tetracaine typically begins working within 3 to 10 minutes and has a duration of action of roughly 30 to 60 minutes. Lidocaine has an onset of action of about 1 to 2 minutes and a duration of action of approximately 15 minutes. The combination provides both rapid onset and extended numbing effects by utilizing the distinct pharmacokinetic properties of each component. Benzocaine and tetracaine are both ethyl esters of p-aminobenzoic acid.

The mechanism of action for these local anesthetics of benzocaine, lidocaine and tetracaine is to reversibly block voltage-gated sodium channels. They prevent sodium from moving into the nerve cells and stop the impulse transmission. These drugs block sodium in a use dependent manner meaning that the higher the firing rate, the more efficacious the drug is in blocking the sodium channel.
