Enclomiphene Citrate Description1-4

Enclomiphene citrate is a nonsteroidal selective estrogen receptor modulator (SERM) derived from clomiphene citrate. While clomiphene is a racemic mixture containing both enclomiphene (the trans-isomer) and zuclomiphene (the cis-isomer), enclomiphene consists solely of the trans-isomer. Due to its ability to stimulate endogenous testosterone production, enclomiphene is being considered for the management of secondary hypogonadism in men, particularly in cases where preservation of fertility is desired.

Mechanism of Action1-4

Enclomiphene works by antagonizing estrogen receptors in the hypothalamus, thereby reducing negative feedback inhibition on the hypothalamic-pituitary-gonadal (HPG) axis. This leads to:

  • Increased GnRH secretion (from the hypothalamus)
  • Stimulation of LH and FSH release (from the anterior pituitary)
  • Subsequent endogenous testosterone production and spermatogenesis (via Leydig and Sertoli cell activation)

Most side effects are mild and transient, but may include:

  • Headache
  • Nausea
  • Mood changes or irritability
  • Gynecomastia or breast tenderness
  • Acne or oily skin
  • Elevated hematocrit or hemoglobin (monitor if long-term use)

Contraindications1-4

  • Known hypersensitivity to enclomiphene or other SERMs
  • Active or previous history of hormone-sensitive cancers (e.g., prostate or breast cancer)
  • Severe liver disease
  • Uncontrolled thromboembolic disorders

Precautions1-4

  • Regular monitoring of serum testosterone, estradiol, LH, and FSH is advised
  • Assess sperm parameters in men using for fertility support
  • Use caution in men with cardiovascular disease or sleep apnea
  • Enclomiphene may increase testosterone beyond physiologic levels if overused
  • Discontinue if vision changes or thromboembolic symptoms occur
  • Store at controlled room temperature (20°C to 25°C / 68°F to 77°F)
  • Protect from moisture and light
  • Keep in a tightly closed container
